In addition, if within the last 2 weeks you have had any of the following, please let us know, we will be happy to reschedule you to a later date:
- Cough, shortness of breath, fatigue, fever, loss of taste, loss of smell, diarrhea
- Travel to high risk area (yourself of someone near you)
- Tested for COVID-19 and are awaiting results
- Tested for COVID-19 result was positive
- You have been in close contact with another person who has been diagnosed with or awaiting results for COVID-19
